I used the sims 2 body shop to create a suitable old lady, and then copied her to Photoshop. Sasha still had my Sims 2 at that point so I couldn't put her in the program and make her bend over patronisingly like you're a little kid. I just made a few edits, background, fixed up the background and dress, and inserted the text.
